INCIDENT REPORTING FORM

Stanwood Camano Little League is committed to providing a positive learning and skill building environment for our players, open communication with our parents, tools for the success of our coaches, and gratitude and visibility for our sponsors. Stanwood Camano Little League is here to build and sustain a long-term love of baseball and softball in our community.

As a part of our dedication to our baseball & softball community, the following reporting form is made available to Members of Stanwood Camano Little League for the purpose of reporting incidents and concerns related to Little League related events such as practices and games. The goal is to offer a secure method of contacting Stanwood Camano Little League whenever a situation arises that warrants additional attention and potential investigation.

Please do not use this system to report emergencies that require immediate attention. Instead, call 911.

IMPORTANT: Incidents reported through this system will be reviewed by a Stanwood Camano Little League Board Member, typically within 24 hours of receipt. This form is strictly for the use of the Stanwood Camano League community and guests. Reports will be addressed as needed and on a case-by-case basis. All reports should remain professional in tone with a goal of resolution in mind. Please use specifics when describing the situation whenever possible.

Please note that reports involving situations such as harassment, intimidation, depression, assault, or someone hurting themselves are very serious and should be addressed by someone who can help. There may be times when this cannot be handled solely by our board of volunteers and we will need to involve the proper authorities. We are mandated reporters.

Please note that anonymous complaints will not result in formal discipline.

Stanwood Camano Little League Incident Reporting Form

UMPIRE INCIDENT REPORTING FORM

This reporting form is intended exclusively for use by SCLL umpires and junior umpires to document incidents that occur before, during, or after games.

Umpires are expected to report any situation that involves:

Ejections of players, coaches, managers, or spectators
Unsportsmanlike conduct or abuse directed toward umpires
Safety concerns or field-related issues
Violations of Little League rules or SCLL policies
Any situation that may require follow-up or disciplinary review

Timely and accurate reporting is critical to ensuring that the league can properly review incidents, support its umpires, and maintain a safe, respectful environment for all participants.

Reports should be submitted as soon as possible following the incident, while details are still fresh. Please include as much relevant information as possible, including teams involved, inning or approximate time of occurrence, individuals involved, and a clear description of what took place.

All reports will be reviewed by the appropriate league officials and handled in accordance with SCLL policies. Umpires may be contacted for additional information if needed.
